The treating pain management doctor at the Houston pain clinic is a Board Certified Anesthesiologist who knows that treatments differ from one patient to another and therefore provides the most aggressive and evidence based pain management therapy possible. In combination with the treatment you may receive as a pain management patient, you may also need our Houston physical therapy services as well.

How Do I Become A  Pain Management Patient At Inclusive Clinics?

Chronic pain management patients are required to have medical records of previous medical history to validate your chief complaint of pain upon arrival. The New Patient Forms may be downloaded by CLICKING HERE.
Please pay close attention to page 16 (medical release form) in the packet. The medical release form can be filled out and faxed to us at (888) 812-4235 or scanned and emailed to us prior to your visit so that we may have time to retrieve your records from the respective entity at records@inclusiveclinics.com.
Certain medical records must be reviewed by our Houston pain management doctors  before an appointment is scheduled for you. These records include recent office visit notes with your primary care physician or other Houston pain doctors who have been treating the condition for which you are requesting an appointment to see us.
We will also need a complete list of current medications, notes from previous surgeries/procedures related to your pain, radiology reports from X-ray, MRI, or CT exams (films are helpful, as well), and insurance information. A referral from your Houston primary care physician is required if deemed necessary by your insurance carrier. Additionally, if you have been treated at a pain facility before, we will need to have those records for review.
Only once we have these records on hand and reviewed will we be able to schedule an appointment for you. No patient will be scheduled for an initial visit without providing all of the appropriate records first.
It is your responsibility to make arrangements to get us the complete list of records; our office staff is not responsible for procuring your medical records although we can attempt to do so once we have your signed and validated medical release form (pg 16 of Patient Registration Packet).
Pain medications (pharmacological treatment) may be a part of your chronic pain or acute Houston pain management treatment at Inclusive Clinics. Pain medicines can be very helpful in allowing you to continue to work, take care of your family and do other things that are important to you. Opioids and other pain medicines are not, however, a substitute for treatment approaches aimed at addressing the pain generator itself, including nerve block injections and physical therapy/physical rehabilitation.
If pain medication is prescribed as part of your treatment, you will be expected to safeguard your medication and comply with the other requirements of your Opioid Agreement located in the New Patient Registration Packet.  In addition, failure to participate in the non-medication components of your treatment can result in discharge form the practice.
Opioid medications are not appropriate for all patients. For that reason there is no guarantee that a patient will receive a prescription(s) for pain medication(s) on their first visit.

Who will provide my medical care?
Inclusive Wellness & Rehabilitation is staffed by a board-certified physician who is skilled in the treatment of non-life-threatening injuries and illnesses. Our physician and our physician extenders are dedicated to providing you excellent medical care in a compassionate, respectful manner within a warm, comforting environment.
How much does an office visit cost?
The initial regular office pain management visit fee is $500.  The initial office visit fee for patients enrolling in our Suboxone treatment program is $300.00, with follow-up visits priced at 150.00.  The frequency of follow-up visits is determined on an individual basis.
What payment methods are accepted? Will insurance pay for my claim?
Payment methods include cash, major credit cards and insurance (please call ahead for insurance verification). We are sorry, but we cannot accept personal checks. We accept out of network insurance benefits from most major medical plans and most automobile insurance companies for injuries that result from automobile accidents.
